Export to newly proposed wide stcm table

This can now be supported by usagi, using the new MappingTypes (Maps to, Maps to Value, Maps to unit, etc.).

Any updates on this new mapping table structure and when it will be available for use with the usagi version? Does not make sense for us to use it at this point until the value_as_string and value_as_concept_id fields are part of the table and the stcm is exported in the new table format.
